Continuity: When Mark tells Tim his little speech in Klingon, you can see the smoke from his cigarette rising up but cannot see the cigarette. The next shot he has his elbow on the table and is holding the cigarette parallel to his eyes. After this shot the cigarette goes back to being out of sight.

Much of the scene with Largeman and the doctor in his office was ad libbed by Ron Leibman. Liebman made up patting Braff on the knee and saying "Hey, you're alive" on the spot, and Braff actually did jump from it. He also did some research by calling other doctors, and threw in the line about taking off his shoes and scratching his feet at the very last moment, something doctors actually do. See more...

Continuity: In a profile shot, Largeman and Sam sit close together in a bathtub talking about Largeman's mom. Largeman begins to tear up and when the film cuts to Sam saying "Largeman" we see she is suddenly leaning against the back of the tub. When it returns to the profile she is kneeling close to him again.
Continuity: Whilst Largeman is saying goodbye to Sam in the airport and says to her "You've changed my life", Sam has her hand on her cheek and you can see she covers up part of her ear. When the shot changes to a closeup of Sam her hand has changed and is further up her head and covering her entire ear. You can also see that her facial expression has changed between shots going from her eyebrows and mouth being clenched to being relaxed.
Continuity: In bathtub scene, when Largeman is telling Sam about the memory that the necklace brings up of his mom, you can see the top of the Dixie cup on the other side of her. Sam hadn't gotten it yet, she will get it later to collect his tear by leaving the tub. When she does this, it is gone. The DVD has deleted shots of this scene that makes it right.
Continuity: When Largeman is eating breakfast after waking up after the party (when he has BALLS written on his forehead) the milk is first turned with the handle facing him, and then in the next shot, the milk is turned with the handle facing away from him, then it is back to facing himin the next shot.
Other: When Largeman is at the restaurant, the first place you see him is in the kitchen. When he exits the kitchen, if you look straight ahead, you can see some doors that open up to reveal the actual kitchen with waiters and cooks. The kitchen we see Largeman in was built as part of the set, and the kitchen we see straight ahead is the actual restaurant's kitchen. This is talked about on the commentary with Zach Braff and the other crew members.
Continuity: In the scene when Largeman is talking to Sam in the doctor's waiting room, she gives him her headphones so he can listen to the Shins' song. In the shot that shows Largeman from the side he listens for a few seconds then starts to give a big smile and look at her, then it cuts back to a front view and he is blank faced again and looking down.
Factual error: The town that they live in can't exist geographically, despite being set in reality. Tim works as a knight for Medieval Times which is off rt 17 near Rutherford. They also mention that Sam's "brother", Tetembe, attends Rutgers which has a New Brunswick, Camden and Newark campus. The closest campus to Rutherford is Newark which is still about 40 minutes away, so in order for the characters to be in a suburban location within 15 or so minutes of both Medieval Times and Rutgers is geographically impossible.
